Massive pleural effusion, what should we do in emergency department? a case report

Indonesia has been known as an endemic country of tuberculosis (TB). Most the cases are pulmonary TB, and pleural effusion is one common cases. Untreated can become massive effusion, a true emergency case in room. In this report we present 21-years-old female patient with new onset due to TB infection. A 21-year-old no previous medical illness came ER shortness breaths since week ago. Cough unmeasured fever have reported month Tachypnea, extreme tachycardia, asymmetric chest movement, decreased vocal fremitus, dullness percussion, left sound were found. Chest x-ray showed tracheal deviation. High flow nasal cannula was given blood gas analysis interpreting moderate respiratory distress. Thoracentesis immediately performed estimated 1200 CC yellow coloured fluid production. Other laboratory findings include hyponatremia hypoalbuminemia. This diagnosed type treated anti-TB drugs, mucolytic, corticosteroid, analgesics. Serial x-rays improvement effusion. developing countries like Indonesia, most causes infection besides malignancy. diagnosis be established history taking, physical examination, sonography, x-ray, and/or CT-scan. must within minutes after established.
